When should wisdom teeth be removed?

Not every wisdom tooth requires removal.

The wisdom teeth are impacted, either partially or fully, and cannot erupt into a functional position. 

Partial eruption creates a flap of gum tissue over the tooth that traps food and bacteria and is a frequent cause of recurring infections called pericoronitis.

The wisdom teeth are causing pain, swelling, jaw stiffness, or pressure against neighbouring teeth. 

These symptoms often indicate an eruption problem or an emerging infection.

There is decay in the wisdom tooth itself or in the second molar adjacent to it, caused by the difficulty of cleaning in the area where the wisdom tooth is crowded against its neighbour.

Cysts or other pathology are developing around the wisdom tooth root, which if left untreated, can cause significant bone damage.

The patient is about to begin orthodontic treatment, and wisdom tooth pressure could compromise the result.

Dr Elzayat evaluates wisdom teeth using panoramic and Cone Beam 3D imaging to assess root position, angulation, and proximity to the inferior alveolar nerve before recommending removal.

Simple vs. surgical wisdom tooth extraction

The complexity of a wisdom tooth extraction depends on how far the tooth has erupted and how it is positioned in the jaw.

A simple extraction is performed when the wisdom tooth has fully erupted above the gum line and can be removed with forceps under local anesthesia, similar to a standard tooth extraction. 

This is the most straightforward type, and recovery is typically quick.

A surgical extraction is required when the wisdom tooth is partially or fully impacted beneath the gum and bone. 

This involves making a small incision in the gum tissue, removing a small amount of bone if needed to access the tooth, and, in many cases, dividing the tooth into sections for easier removal.

 The site is then cleaned and closed with sutures. 

Surgical extractions are performed under local anesthesia, and sedation options are available.

Sedation options for wisdom teeth removal

Oral sedation and IV sedation options may be available depending on the complexity of the case. 

Patients who receive sedation will need a responsible adult to drive them home after the procedure.

Recovery after wisdom Teeth Removal

Most patients recover from wisdom teeth removal within three to five days for simple extractions and five to seven days for surgical cases. 

Swelling, mild discomfort, and restricted jaw opening are normal in the first few days and are managed well with prescribed or over-the-counter pain relief and ice packs applied to the outside of the face.

Key aftercare instructions include avoiding sucking through straws, smoking, or spitting forcefully in the first 24 to 48 hours to protect the blood clot that forms in the socket. 

Dislodging this clot causes a painful condition called dry socket that delays healing significantly.

Soft foods are recommended for the first few days. Most patients return to normal activity within a few days and feel fully recovered within one to two weeks.

Should I get all Four Wisdom Teeth removed at once?

Removing all four wisdom teeth in a single appointment is generally recommended when all four require extraction. 

Having a single procedure and a single recovery period is more convenient than spreading the removals across multiple appointments with multiple recoveries. 

If sedation is being used, a single appointment is particularly practical.

However, if only one or two wisdom teeth are problematic, Dr. Elzayat will recommend removing only those that require attention rather than extracting teeth that are erupting normally and causing no issues.
